| Permanent Identification (P1A) | This form is only to be used by veterinarians and authorised identifiers to identify microchip numbers when implanting them, as required under the Companion Animals Act 1998. | Verification of Existing Microchip (M1) | This form must be filled out by veterinarians and authorised identifiers to verify the microchip number of an animal which is already microchipped, as required under the Companion Animals Act 1998.
